Physical Therapist Job Description

We are seeking a highly skilled Physical Therapist to join our Home Health team in the Seattle area. In this role, you will work closely with patients and their families to develop and implement personalized therapeutic plans, promoting optimal patient outcomes and improving quality of life.

Key Responsibilities
  • Conduct thorough assessments and create comprehensive treatment plans in collaboration with patients and healthcare professionals.
  • Provide high-quality physical therapy interventions to enhance patient mobility, function, and independence.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Bachelor's or Master's Degree from an accredited Physical Therapy Program.
  • Oregon Physical Therapist License (or ability to obtain within 6 months).
  • At least 1 year of experience in Home Health or a similar healthcare setting (or equivalent education).
