About the Role

We are seeking a skilled Care Coordinator to join our team at UnitedHealthcare. As a key member of our healthcare team, you will play a vital role in ensuring the best possible care for our patients.

Key Responsibilities
  • Assess and Plan Care: Develop and implement individualized care strategies that meet the unique needs of each patient.
  • Coordinate Care Services: Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage Care Plans: Serve as a single point of contact for patients, managing care plans throughout the continuum of care.
  • Communicate Effectively: Share health-related information with all stakeholders to ensure quality coordinated care and services are provided promptly to all members.
  • Advocate for Patients: Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
Requirements
  • Current RN License: Possess a current, unrestricted RN license in the state of Hawaii.
  • Clinical Experience: Have at least 2 years of clinical experience.
  • Long-Term Care Experience: Have experience working directly or collaborating with services for long-term care, home health, hospice, public health, or assisted living.
  • Technical Skills: Possess an intermediate level of proficiency with MS Word, Excel, and Outlook.
  • Travel Requirements: Ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
Preferred Qualifications
  • Bachelor's Degree: Possess a Bachelor's degree or higher.
  • CCM Certification: Hold a CCM (Certified Case Manager) certification.
  • Discharge Planning Experience: Have experience with or exposure to discharge planning.
  • Utilization Review Experience: Have experience with utilization review, concurrent review, and/or risk management.
  • Electronic Charting Experience: Have experience with electronic charting.
  • Community Resource Experience: Have experience arranging community resources.
  • Field-Based Work Experience: Have experience working in a field-based setting.
  • Case Management Experience: Have experience in case management.
  • Community Health Experience: Have experience working within the community health setting.
  • Cultural Competence: Possess a background or experience working with diverse populations, including those with complex medical or behavioral needs.
