Health and Social Services Coordinator Role
At UnitedHealthcare, we're committed to simplifying the healthcare experience and creating healthier communities. As a Health and Social Services Coordinator, you'll play a vital role in coordinating the long-term care needs for patients in the local community.
Key Responsibilities:
- Assess, plan, and implement care strategies tailored to individual patient needs.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Manage the care plan throughout the continuum of care as a single point of contact.
- Communicate with stakeholders to ensure quality coordinated care and services are provided efficiently to all members.
- Advocate for patients and families as needed to ensure their needs and choices are represented and supported by the healthcare team.
Requirements:
- Current, unrestricted Social Worker license or LMHC in the state of Hawaii.
- 2+ years of social work or similar experience.
- Experience working directly or collaborating services for long-term care, home health, hospice, public health, or assisted living.
- Intermediate experience working with MS Word, Excel, and Outlook.
- Ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
Preferred Qualifications:
- CCM (Certified Case Manager) certification.
- Case Management experience.
- Experience with or exposure to discharge planning.
- Experience with utilization review, concurrent review, and/or risk management.
- Experience with electronic charting.
- Experience with arranging community resources.
- Field-based work experience.
- Background in managing populations with complex medical or behavioral needs.
- Background in mental health or experience working with serious mental illness.
- Bilingual in Cantonese or Mandarin.
