Position Overview:
- Facilitate access to comprehensive health care services for eligible individuals, particularly pregnant women and children aged 0-5.
- Concentrate on intensive management of care services tailored for specific demographics.
Required Education and Experience:
- Bachelor's degree in social work or a related human services discipline.
- Preferred: Two years of relevant experience.
Licensure or Certification: Must possess a valid NC driver's license.
Key Responsibilities:
- Collaborate with healthcare providers and agencies to develop patient-centered care strategies.
- Execute care management processes aimed at enhancing patient outcomes.
- Evaluate patients to determine the appropriate level of care management services required.
- Build and sustain relationships with medical homes and healthcare providers.
- Oversee crisis prevention and stabilization efforts for patients.
