Position Overview:

  • Facilitate access to comprehensive health care services for eligible individuals, particularly pregnant women and children aged 0-5.
  • Concentrate on intensive management of care services tailored for specific demographics.

Required Education and Experience:

  • Bachelor's degree in social work or a related human services discipline.
  • Preferred: Two years of relevant experience.

Licensure or Certification: Must possess a valid NC driver's license.

Key Responsibilities:

  • Collaborate with healthcare providers and agencies to develop patient-centered care strategies.
  • Execute care management processes aimed at enhancing patient outcomes.
  • Evaluate patients to determine the appropriate level of care management services required.
  • Build and sustain relationships with medical homes and healthcare providers.
