Job Title: Registered Nurse

At Outagamie County, we are seeking a dedicated and compassionate Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in providing person-directed care to our residents, ensuring their medical and social needs are met.

Key Responsibilities:
  • Provide person-directed care through planning, administering, and reviewing resident care via care plans, physician orders, nursing orders, and resident choice.
  • Assess residents' medical and social needs, organize notes, gather documents, and assist MD during acute care rounds.
  • Report and document changes in condition according to policy.
  • Report infections to Infection Control/Skin Care Coordinator and set up precautions as needed.
  • Perform supervisory functions of the Neighborhood Manager and Registered Nurse Specialist as delegated. Supervise, direct, and evaluate the performance of Nursing Assistants.
  • Educate nursing team members and household team members as appropriate regarding the implementation of policies and procedures.
  • Attend nurse meetings and neighborhood/household meetings, household council meetings, community circles, huddles, and care conferences.
  • Ensure all households participate in Quality Improvement and take appropriate follow-up to assure quality outcomes. Perform professional audits and observations to confirm the competency of all household team members. Participate in quality assurance/performance improvement committees as delegated.
  • Promote empowerment of the team in providing person-directed living by providing leadership and vision to the household and serving as a role model. Participate in team problem-solving, monitor team members' performance, and provide feedback through coaching and counseling.
  • Maintain a safe, comfortable, and functional environment by assessing the household for potential hazards, implementing practices utilizing appropriate techniques regarding sanitation, infection control, fire safety, disaster preparedness, and emergency care. Investigate and report all incidents and accidents, and observe the team for safe work habits.
Requirements:
  • Degree in Nursing.
  • Current State of WI Nursing License.
  • Maintain current certification in adult CPR.
Preferred Skills:
  • Ability to make use of common medical and nursing equipment.
  • Ability to assign, supervise, and review the work of others.
  • Ability to comprehend and interpret a variety of documents, including medical reports and charts, x-rays, lab reports, policy and procedure manuals, medical references, etc.
  • Ability to prepare a variety of documents, including lab sheets, chart reviews, incident reports, care plans, etc.
  • Ability to use and interpret medical terminology.
  • Ability to communicate effectively with physicians, team members, pharmacy, residents, and their families/legal representatives, and others verbally and in writing.
  • Ability to assure that federal, state, and local laws, as well as accepted professional standards and principles of professional organization, boards, and councils, are followed.
  • Ability to see person-directed living as an important component in providing residents with a sense of health and well-being.
  • Ability to understand the importance of modeling positive attitudes and behaviors.
