Job Summary:
Sharecare is seeking a highly skilled Advanced Diabetes Management Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to optimize individual and population-wide well-being.
Key Responsibilities:
- Provide comprehensive diabetes care to patients with glycemic dysfunction, including assessment, intervention planning, and implementation.
- Collaborate with attending physicians to assure the achievement of performance measures for the diabetes population.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide education and guidance to patients and their families on diabetes management, including medication adjustment, medical nutrition therapy, and exercise planning.
- Initiate and interpret diagnostic tests and procedures relevant to the patient's current status.
- Order and/or adjust frequency of blood glucose monitoring and diabetes-related labs.
Requirements:
- Master's Degree in Acute Care Nurse Practitioner from an accredited school of nursing.
- At least two (2) years' experience as a registered nurse or as a nurse practitioner.
- Board-certified in Advanced Diabetes Management or take the exam when eligible.
- Current certification of fitness from Board of Nursing as a nurse practitioner.
- B.C.L.S. certification required.
