Key Responsibilities

The Communication Specialist is responsible for delivering speech therapy services as part of the overall personal care provided to patients. In this role, you will:

  • Conduct assessments and evaluations to identify patient needs and collaboratively establish goals for the care plan
  • Deliver speech therapy services, including assessments, evaluations, procedures, and educational activities as specified in the care plan
  • Communicate patient progress to the Clinical Supervisor and referring Physician
  • Maintain accurate documentation of daily changes and updates to patient care plans, including discharge planning
  • Participate in staff meetings, team discussions, and educational sessions as required by the agency
  • Complete all OASIS time point assessments in accordance with Medicare criteria and submit recertification documentation as per established policies
Qualifications
  • A valid and unrestricted State Professional License
  • Current driver's license and proof of insurance
  • Minimum of one year of experience as a professional Speech Language Pathologist in a home care setting, with competence in comprehensive assessments and OASIS-C
  • Ability to exercise sound clinical judgment
  • Capability to assess and document patient needs, formulating individualized care plans accordingly
  • Proficiency in clinical skills
  • Familiarity with Microsoft Office applications
