Job Title: Youth Mental Health Case Manager

Job Summary:

The Youth Mental Health Case Manager provides evidence-based services to children and their families to maintain or improve their ability to function in the community and work towards their recovery goals.

Key Responsibilities:

  • Assist youth with mental illness and/or behavioral issues and their families in accessing medical, social, educational, and other appropriate services.
  • Encourage youth to live their best lives through support, resources, skills training, parent training, safety planning, crisis intervention, and other services.
  • Work in collaboration with other agencies within the community and state to link youth and their families to other needed services.
  • Provide monitoring, assessment, service planning, and coordination, as well as skills training and development.
  • Work independently, under limited supervision, reporting major activities through periodic meetings.

Requirements:

  • Bachelor's degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
  • Previous experience in the field of mental health or social services preferred.
  • Must maintain a valid Texas Driver's license, auto liability insurance, and a driving record acceptable to PermiaCare's insurance requirements.
  • Required to pass criminal history and background checks as well as pre-employment drug screen.
  • Must obtain QMHP certification within 6 months of hire.

Working Conditions:

This position may require travel to agency program sites, community, and residential sites, and/or locations outside the PermiaCare catchment area. This position may require transport of agency individuals and/or individuals served.
