We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Ochsner Home Health of Raceland. As a valued member of our healthcare team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

The Licensed Practical Nurse (LPN) in Home Health is responsible for providing high-quality patient care and performing technical skilled care in the patient's home. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide services within the scope of practice as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Instruct patients and their families in the care of the patient and maintenance of a healthy environment for the patient.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
Requirements
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
