Clinical Social Worker

Job Summary:

We are seeking a highly skilled Clinical Social Worker to join our team at 22nd Century Technologies. As a Clinical Social Worker, you will play a critical role in providing social work services to patients and families, coordinating care, and assisting with research protocols.

Key Responsibilities:
  • Assist in coordinating and planning programs to meet the social and emotional needs of patients and their families.
  • Develop treatment plans based on assessments of psychological, social, and medical realities.
  • Collaborate with staff on treatment modalities, including crisis intervention, individual, family, or group psychotherapy.
  • Develop and implement discharge plans for patients with complex medical and psychiatric illness.
  • Identify, make recommendations, and provide referral to community resources to facilitate continuity of patient care.
  • Screen research participants for psychosocial risk and refer appropriately to professional social work staff.
  • Obtain information about research participants' household, living situation, employment, financial situation, and other pertinent information.
  • Communicate with insurance companies and healthcare facilities to identify possible resources for continuity of care.
  • Provide family supportive and crisis management services as needed.
Requirements:
  • Master's degree in Social Work (MSW) or related field.
  • Licensure for independent clinical social work practice.
  • Accredited Master of Social Work (MSW) degree.
  • Medical malpractice insurance.
  • Medical/Clinical Social Work experience.
  • Advanced Clinical License to Practice Independently.
  • LCSW-C for MD, and LICSW for DC or an LCSW for VA.
  • Credentialing process must result in Tier III level of independent social work practice.
Preferred Qualifications:
  • Experience in a medical setting, coordinating and collaborating with medical teams.
  • Strong diagnostic and clinical skills, professional knowledge of and specialized experience in clinical social work.
  • Knowledge of mental health, medical, and social services available in the metropolitan community and throughout the United States and worldwide.
  • Strong interpersonal skills and ability to work effectively with a team.
  • Strong organizational skills with the ability to successfully manage competing priorities.
  • Resourcefulness in problem-solving.
  • Strong customer service focus.
  • Proficiency with MS Suite.
