Key Responsibilities
  • Collaborate with the Case Management Coordinator - RN and other clinical team members to provide comprehensive patient care.
  • Assess patient needs and develop individualized care plans to address social, emotional, and environmental factors affecting patient health.
  • Provide counseling and support to patients and their families to promote healthy coping mechanisms and improve overall well-being.
  • Work closely with healthcare providers to ensure seamless transitions of care and optimal patient outcomes.
  • Participate in multidisciplinary teams to develop and implement policies and procedures that promote patient-centered care.
Requirements
  • Master's Degree in Social Work (MSW) from an accredited program.
  • Current Maryland licensure as a Social Worker (LCSW) is required; certification in case management is desirable.
  • Experience in a clinical setting as a hospital discharge planner is preferred.
