Critical Care Transition Expert

TotalMed is seeking an experienced Critical Care Transition Expert to join our team. As a valued member of our healthcare team, you will provide high-quality care to patients transitioning from critical care to general medical-surgical units. With a strong focus on patient-centered care, you will work collaboratively with the interdisciplinary team to ensure comprehensive care and optimal outcomes.

Your responsibilities will include:
• Assessing and monitoring patients' vital signs, cardiac rhythms, and respiratory status
• Administering medications and performing procedures as needed
• Educating patients and families on post-intensive care recovery, medication management, and self-care techniques
• Communicating effectively with healthcare team members to facilitate patient progress and optimize outcomes
• $20,941 / 13 weeks (approximately $2,063 per week)
• Hampton, VA area (freestanding opportunities available)
Requirements
• Current RN license
• BLS certification (other certifications may be required by facility)
• Two years of recent Step Down experience
• Strong communication and adaptability skills
