Mental Health Therapist

UnityPoint Health is seeking a skilled Mental Health Therapist to join our behavioral health care management team. As a key member of our team, you will integrate the care of individuals within our Behavioral Health Services, contributing professional and clinical knowledge and skills to support patient and family access to behavioral health services.
Responsibilities- Perform comprehensive psychosocial assessments to identify priority needs, strengths, patient preferences, and barriers to care.
- Provide brief crisis intervention services and emotional support to patients and families to enhance their ability to cope with the impact of health conditions.
- Assess grief issues and offer bereavement support.
- Assist with planning for care transitions and collaborate with UPH, community services, and facilities to support patient safety and continuity of care.
- Document assessments, interventions, and referrals in the electronic health record according to documentation standards.
- Assist in identifying the correct level of care for patients and locating appropriate placement.
- Identify patient transitional needs by assessing psychosocial, environmental, financial, and cultural strengths and barriers.
- Maintain comprehensive knowledge of community resources and act as a liaison to refer patients and families to health and social services, health insurance, public assistance, and other resources to meet patient-identified needs.
- Provide expertise and play a key role with the care team in establishing patient-centered goals of care and identifying psychosocial and behavioral strengths and barriers.
- Contribute to the comprehensive, longitudinal plan of care based on patient-centric goals and coping strategies.
- Facilitate and/or participate in interdisciplinary team meetings to review and revise the patient plan of care.
- Collaborate with social workers and other professionals across the continuum and in the community to ensure continuity of care.
- Assume accountability for the development and implementation of an effective discharge plan for complex-care patients.
- Master's degree in Social Work, Professional Counseling, or Marriage and Family Therapy with a clinical/mental health concentration from an accredited school.
- Applicants should have completed practicum experiences as part of their accredited program.
- Emergency Department or Integrated Mental Health Clinic experience.
