About the Role

The Oncology Clinical Pharmacist Expert plays a vital role in delivering pharmaceutical care for patients with oncologic, hematologic, and immunologic conditions in an outpatient setting. This position involves extensive collaboration with healthcare providers within the Cancer Service Line, engaging in drug therapy consultations, participating in clinical research initiatives, and providing education to patients, families, and healthcare staff. Additionally, this role includes mentoring pharmacy residents and students, making it a key clinical position within our Specialty Pharmacy program.

Key Responsibilities:

  • Analyze and interpret medication orders from physicians, assess adverse drug reactions, evaluate therapeutic efficacy, and review patient health status along with their medical history for potential drug interactions.
  • Deliver personalized patient education regarding medication therapy based on individual needs.
  • Collaborate with pharmacy colleagues and technicians in the Specialty Pharmacy, assisting with prescription processing and addressing patient inquiries effectively.
  • Monitor and evaluate drug therapy outcomes for designated patients according to established protocols.
  • Oversee supportive care plans focusing on managing symptoms such as pain, neuropathy, nausea, and vomiting.
  • Develop and update evidence-based treatment guidelines to ensure safe and effective medication regimens.
  • Engage in quality assurance and improvement activities to maintain accreditation standards and comply with legal and regulatory requirements.
  • Represent the Pharmacy Services Department on various committees as assigned.

Qualifications:

  • Doctor of Pharmacy (PharmD) from an ACPE-accredited institution is required.
  • Completion of an ASHP-accredited PGY1 Pharmacy Practice Residency or equivalent clinical pharmacy experience (minimum of 3 years).
  • Completion of an ASHP-accredited PGY2 Oncology Pharmacy Residency or equivalent experience (minimum of 3 years in oncology pharmacy practice, in addition to PGY1 residency).
  • Preferred experience in outpatient oncology pharmacy practice.
  • Board Certification in Oncology Pharmacy (BCOP) is required within two years of employment.
  • Current licensure in the State of Delaware or eligibility for reciprocity.
  • Additional state licensure may be necessary for providing virtual patient care.
