About the Role

We are seeking a highly skilled Clinical Pharmacist to join our Inpatient Pharmacy team at ChristianaCare. As a Clinical Pharmacist, you will play a critical role in providing patient-centered care and contributing to the delivery of high-quality pharmacy services.

Key Responsibilities
  • Participate in direct patient care through daily interdisciplinary rounds, collaborating with healthcare teams to optimize medication therapy and improve patient outcomes.
  • Manage specific medication therapies, including warfarin, aminoglycosides, vancomycin, IV to PO conversions, renal dose adjustments, and indication-directed dosing adjustments.
  • Evaluate drug efficacy, monitor therapeutic goals and outcomes, and review for contraindications/precautions.
  • Order and evaluate laboratory tests per approved protocols to support medication therapy monitoring.
  • Identify opportunities for counseling and medication history completion during chart review.
  • Communicate recommendations directly with providers and electronically document interventions as appropriate.
  • Assist with projects such as Medication Use Evaluations (MUEs), drug monographs, and performance improvement activities.
  • Review and interpret provider orders, dispense medication, and coordinate and supervise workplace activities.
Requirements
  • Bachelor of Science degree in Pharmacy or PharmD degree from an accredited College of Pharmacy required.
  • ASHP-accredited pharmacy residency preferred or equivalent experience (3 years acute care).
  • Board certification preferred.
  • Current licensure (or eligible to reciprocate) in the State of Delaware.
