Hospice Nurse

Overview:

We are seeking a compassionate and skilled Hospice Nurse committed to delivering exceptional end-of-life care. This role involves providing quality patient care in a hospice environment, working closely with a multidisciplinary team to ensure the comfort and dignity of patients during their final stages of life.

Key Responsibilities:

  • Perform comprehensive patient assessments and create individualized care plans that address specific needs.
  • Administer prescribed medications, treatments, and IV infusions with precision.
  • Monitor vital signs, symptoms, and pain levels, and report any significant changes promptly.
  • Conduct specimen collection and assist in maintaining sterile processing protocols.
  • Implement infection control measures and provide care for infectious diseases.
  • Deliver urgent care interventions as necessary and manage durable medical equipment (DME).
  • Provide ostomy care, memory care, and perform physical examinations as needed.
  • Collaborate effectively with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.

Qualifications:

  • Active RN license in the state of practice.
  • At least 2 years of experience in a clinical or hospice care setting.
  • Proficient in ICD9 coding, patient assessment, and care planning.
  • Strong understanding of medical terminology, procedures, and sterile processing techniques.
  • Excellent communication skills with a compassionate approach to patient and family interactions.

Preferred Skills:

  • Experience with IV infusion therapy.
  • Familiarity with electronic medical record systems such as eClinicalWorks.
  • Proficiency in venipuncture for blood draws and IV therapy.
  • Prior experience in memory care practices.
