About the Role

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Hospice team as an On-Call RN. As a key member of our care team, you will provide high-quality patient care and support to patients and their families in their homes.

Responsibilities
  • Perform initial nursing assessments on new admissions
  • Develop, implement, and revise individualized patient care plans in collaboration with the care team
  • Regularly communicate patient progress to the clinical manager, physician, and care team
  • Record patient progress, pain management, and symptom management changes/outcomes
  • Educate families on patient condition, needs, and care
  • Make referrals to other hospice care disciplines as needed or document rationale for not doing so
  • Perform on-call responsibilities and services to patients/families as assigned
Requirements
  • Current RN license, specific to the state(s) you are assigned to work
  • One+ year of nursing experience required
  • Current CPR certification
  • Valid driver's license, reliable transportation, and liability insurance
