About the Role

This full-time position offers a unique opportunity to work in a dynamic and collaborative environment, providing comprehensive care to patients and families with complex healthcare needs. As a Registered Nurse, you will be responsible for:

  • Providing high-quality patient care, ensuring safety, effectiveness, patient-centeredness, timeliness, efficiency, and equity.
  • Collaborating with interdisciplinary teams to develop and implement individualized care plans.
  • Performing systematic assessments, prioritizing, synthesizing, and documenting patient data.
  • Implementing plans of care, delegating tasks, and monitoring the work of other nurses and unlicensed staff.
  • Developing and implementing teaching plans to promote patient education and empowerment.
  • Exercising judgment in coordinating care, referring patients to other services or healthcare professionals as needed.
  • Performing on-site and telephone triage, using best practice guidelines and documentation to ensure optimal patient outcomes.

Requirements

To be considered for this role, you must possess:

  • A Bachelor's degree in nursing, Associate's degree in nursing, or diploma from an accredited nursing school.
  • Current licensure as a Registered Nurse in the state of Texas.
  • Experience working in ambulatory care or a related field.
