Job Overview

We are seeking a dedicated Community Outreach Specialist to join our team at AccessCNY INC. As a vital member of our organization, you will play a key role in providing support and services to individuals with disabilities.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and community partners.

A valid driver's license and reliable transportation are required for this position.

Salary: $18.05-$18.56 per hour.

Responsibilities:

  • Make home visits to provide information, support, and assist with acquiring services
  • Provide information, advocacy, and referral services to individuals and families
  • Assist with the eligibility process through OPWDD
  • Complete documentation based on program requirements
  • Research community resources and provide linkage to increase inclusion

Requirements:

  • High school diploma/GED plus 3 years' experience working with individuals with disabilities
  • Associate's degree in Human Service field plus 2 years' experience preferred
  • Basic word processing and e-mail skills required
