Job Title: LPN Visit Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Care Visit Nurse. As a LPN Visit Nurse, you will provide high-quality patient care in the comfort of patients' homes, working closely with our interdisciplinary team to deliver exceptional care and support.

Responsibilities:
  • Provide direct nursing care to patients in their homes, as prescribed by physicians
  • Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care
  • Work collaboratively with the healthcare team to develop and implement individualized care plans
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
Requirements:
  • Current LPN licensure in the state of practice
  • Current driver's license, vehicle insurance, and access to a dependable vehicle
  • CPR certification
  • Previous home care experience preferred
