Responsibilities:

  • Conduct thorough assessments of clients' situations by interviewing and reviewing their personal history
  • Develop and implement individualized treatment plans to address clients' unique needs
  • Establish and track client progress milestones to ensure effective care
  • Communicate client updates to relevant parties in a timely and professional manner
  • Facilitate referrals to other healthcare professionals and programs as needed
  • Maintain accurate and up-to-date client documentation

Qualifications:

  • Previous experience in social work, counseling, or related fields
  • Compassionate and empathetic demeanor
