About the Role

Catholic Charities is seeking a dedicated and compassionate Case Manager to join our Families of Promise Program in Lake County. As a key member of our team, you will work closely with youth and their caregivers to address unmet needs and provide support during a challenging time.

Key Responsibilities
  • Under the direction of our program supervisor, ensure that your work aligns with the mission, vision, and values of Catholic Charities.
  • Collaborate with children, caregivers, and parents to identify and address barriers to stability and well-being.
  • Provide direct service in the office, via telehealth, or in the field/homes, as needed.
  • Develop and implement interventions, monitor progress, and reassess needs to ensure successful outcomes.
  • Assist clients in accessing community resources and supports, while partnering with local agencies and organizations.
  • Advocate on behalf of clients to ensure their needs are met.
  • Maintain accurate and timely records in our electronic medical record system and administrative reports.
  • Participate in clinical and administrative staff meetings, as well as multi-discipline team meetings.
  • Review treatment plans and progress with your supervisor to ensure continued support and guidance.
