Job Title: Home Health Therapist

BAYADA Home Health Care is seeking a skilled Home Health Therapist to join our team in Center Rutland, Vermont.

Job Summary

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Therapist, you will work independently to assess and develop individualized care plans, implement interventions, and educate clients and their families on rehabilitative care and activities.

Responsibilities
  • Conduct home visits to assess clients' needs and develop care plans.
  • Implement manual therapeutic exercises, gait training, balance, and other interventions to promote clients' health and safety.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
  • Accurately document observations, interventions, and evaluations in a state-of-the-art touch pad tablet.
Qualifications
  • A current Vermont Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
