Key Responsibilities:
  • Conduct evaluations to assess patients' status in terms of addiction recovery and overall mental and physical health.
  • Formulate diagnosis based on assessment findings and develop individualized treatment plans.
  • Collaborate with Holon Health's care team, external medical professionals, and judicial points of contact to provide whole-patient care and coordinate treatment plans.
  • Prescribe and monitor medications, ensuring appropriate dosage, potential side effects, and interactions with other medications.
  • Provide patient education on health conditions, treatment options, medication management, and self-care strategies to promote overall wellbeing.
Requirements:
  • Active and unrestricted Indiana license as a Nurse Practitioner, Physician Assistant, or Medical Doctor/Doctor of Osteopathy.
  • Active DEA in states in which you hold a license; CDS in states where applicable.
  • Several years of experience in the practice of addiction medicine or psychiatry.
  • Strong affinity with and compassion for people with SUD.
  • Excellent interpersonal skills.
