We are seeking a highly skilled and compassionate Assertive Community Treatment (ACT) Case Manager to join our vibrant and interdisciplinary team serving adults with serious mental illness in the greater Portland area. As a key member of our team, you will provide comprehensive care and support to our clients, working collaboratively with psychiatrists, nurses, and other professionals to ensure the best possible outcomes.
Key Responsibilities:
- Conduct thorough intake and assessment of clients, developing individualized plans of care in collaboration with the treatment team.
- Facilitate regular team meetings and ensure timely implementation of treatment plans, monitoring progress and making adjustments as needed.
- Build strong relationships with clients and their families, providing education and support to help them navigate the mental health system.
- Collaborate with community resources and providers to ensure seamless care coordination and access to necessary services.
- Participate in ongoing training and professional development to stay current on best practices in ACT and mental health treatment.
Requirements:
- MHRT-C certification or eligibility, or a Certified Rehabilitation Counselor or Employment Specialist, or a Certified or Licensed Alcohol and Drug Counselor.
- Bachelor's degree from an accredited four-year institution of higher learning preferred.
