Position Overview

Essential Information About the Care Transition Specialist Role:

  • This is a full-time daytime role within our Hospice House.
  • Work schedule is Monday through Friday, with options for 8-hour shifts or 4/10-hour days, requiring flexibility to accommodate unit needs.

Role Summary:

The Care Transition Specialist is responsible for accurately managing data and information related to patient transfers and discharges within the Hospice House.

Qualifications:

For Registered Nurses (RNs): Must be a graduate of an accredited nursing program, with an Associate of Science in Nursing (ASN) or a Bachelor of Science in Nursing (BSN) preferred. A valid Registered Nurse license in the State of Ohio is required, free from any board actions. A minimum of two years of acute care nursing experience is preferred.

For Licensed Practical Nurses (LPNs): Must hold a valid LPN license in the State of Ohio, free from any board actions. One year of nursing experience is preferred.

  • Current CPR certification is required.
  • Certification in Hospice & Palliative Medicine (CHPN) is preferred and must be obtained within two years of employment (eligibility requires completion of 500 hours in hospice/palliative care within the last 12 months).
  • Proficient computer skills for accurate documentation of services and care are necessary.
  • Must possess a valid Driver's License with a safe driving record and meet state minimum automobile insurance coverage.

Key Responsibilities:

  • Engage with the Medical Director and staff to gather precise data for scheduled patient discharges and transfers.
  • Work collaboratively with caregivers and patients to assess needs during transfers, ensuring continuity of care at the appropriate locations.
  • Provide comprehensive medication information to caregivers and patients, ensuring understanding of medication purposes and delivery.
  • Coordinate with the Medical Director to ensure all prescriptions are prepared and available for patients at the time of transfer, liaising with pharmacies as needed.
  • Order necessary Durable Medical Equipment (DME) for transfer or discharge locations in collaboration with nursing and social work teams.
  • Document physician orders in the electronic health record to maintain accurate information and ensure safe service delivery.
  • Assist in various activities and departments as needed to ensure comprehensive patient care for all Hospice patients.
  • Participate in Continuous Quality Improvement initiatives to enhance the quality of services provided by Ohio's Hospice.
