Job Summary

We are seeking a highly skilled and compassionate LPN to join our team as a Resident Care Supervisor. As a key member of our care team, you will be responsible for providing exceptional care and support to our residents, while also supervising and mentoring our care staff.

Key Responsibilities
  • Provide direct care to residents, including administering medications, performing vital signs, and assisting with daily living activities.
  • Supervise and mentor care staff, providing guidance and support to ensure high-quality care is delivered.
  • Conduct regular rounds to ensure the well-being of residents and identify any areas for improvement.
  • Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Monitor and report any changes in resident condition, including falls, injuries, or illnesses.
  • Participate in quality improvement initiatives to enhance the overall care experience.
Requirements
  • Current LPN licensure in the state of New York.
  • At least one year of experience in a care-giving capacity, with prior medication management experience and supervisory experience desired.
  • Strong leadership and communication skills, with the ability to work effectively with residents, families, and care staff.
  • Ability to lift 30 pounds without assistance and stand for prolonged periods.
  • High school diploma or GED preferred.
