Allegria Nursing and Rehabilitation Center of Port Jefferson is seeking a skilled Physical Therapist to join our team.
Job Summary:We are looking for a highly motivated and experienced Physical Therapist to provide high-quality rehabilitation services to our patients. The successful candidate will be responsible for assessing patients, developing and implementing treatment plans, and providing education to patients and their families.
Responsibilities:- Assess patients to identify their medical condition, functional capabilities, limitations, and potential for rehabilitation.
- Develop and implement treatment plans to achieve specific goals, under the direction of the physician.
- Provide direct patient care, including physical therapy interventions and education.
- Oversee and supervise Physical Therapy Assistants.
- Document and record patient progress, including evaluations, weekly progress notes, recertification, and discharge documentation.
- Ensure compliance with confidentiality, HIPAA, and federal residents' rights.
- Graduation from an accredited Physical Therapy college or university.
- Valid professional license and registration for the state of practice.
- Experience with long-term care and/or geriatric care is preferred.
- Excellent communication and documentation skills.
- Ability to work independently and as part of a team.
