Clinical Care Specialist

We are seeking a skilled Clinical Care Specialist to join our team at BAYADA Home Health Care in Ashland, Massachusetts. This is a full-time position with a competitive salary and comprehensive benefits package.
Salary: $65,000 - $80,000 per year, depending on experience.
In this role, you will work directly with patients in their homes, providing high-quality medical care and support. You will be part of a dynamic team that prioritizes patient-centered care and community engagement.
Key Responsibilities:
- Provide direct patient care, including administering medications, wound care, and other medical interventions.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Conduct home safety assessments and provide education to patients and families on fall prevention, mobility, and other topics.
- Maintain accurate records and communicate effectively with colleagues and stakeholders.
Requirements:
- Bachelor's degree in Nursing (BSN) or related field.
- Current RN licensure in Massachusetts.
- 2+ years of experience in home health care or a related setting.
- Strong communication and interpersonal skills.
- Clinical knowledge and expertise in adult and geriatric care.
