Licensed Practical Nurse

Genacross Lutheran Services is seeking a dedicated Licensed Practical Nurse to join our team in the Assisted Living Center. As a Licensed Practical Nurse, you will provide high-quality care to our residents, working closely with the Assisted Living Manager and Resident Aides to ensure effective care is provided.
Responsibilities- Provide primary direction to Resident Aides on assigned unit/floor/wing to ensure effective care is provided to residents.
- Prepare and administer medications and treatments to residents.
- Perform assessments, including vitals, intakes, and outputs.
- Perform direct care functions, assisting with ADLs and ensuring residents' spiritual, emotional, and physical needs are met.
- Process physician's orders, labs, x-rays, and seek clarification when necessary.
- Communicate effectively with physicians, pharmacists, family members, and other stakeholders to discuss ongoing care of residents.
- Process new resident admissions and communicate admission information to department heads, as appropriate.
- Complete paperwork, medical forms, charting, and all required documentation as part of the resident care process and transition of care process.
- Assist in maintaining infection control programs, policies, and procedures.
- Report allegations of abuse/neglect and report, as appropriate, to proper organization leadership.
- Report hazardous incidents, including injuries, equipment, and other safety concerns, to the appropriate Manager/Supervisor.
- Implement and follow disaster plans in the event of an emergency.
- Participate in committees as assigned.
- Current State of Ohio LPN License.
- 1-2 years of experience in long-term care setting preferred.
- Familiarity with basic computer skills.
- Ability to learn new software applications as required by the position.
- Excellent oral and written communication skills, including the ability to interact with senior executives, managers, supervisors, and direct care staff, as well as residents, families, and physicians.
- Understanding of directions, communication, and response to inquiries.
