Job Summary: CNY Family Care is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to support patient triage and administrative functions within our dynamic practice. Following a 3-month on-site training period, this role will transition to a remote work setting.
Key Responsibilities:
- Address patient inquiries and medical advice requests through the patient portal.
- Manage patient phone calls, providing timely triage and direct communication.
- Review and respond to various patient documents, including surgical records, imaging results, and immunization records.
- Update patient information in the Electronic Medical Record system based on document reviews.
- Communicate significant health status changes to patients' Primary Care Physicians.
Qualifications:
Applicants must possess a valid NYS Nurse Practitioner or Physician Assistant license. A minimum of two years of experience in a family practice or urgent care environment is required. Familiarity with Medent (EMR) is preferred. Candidates should demonstrate energy, a strong understanding of chronic disease management, and excellent critical thinking abilities.
