Job Summary:

The LVN-PACE Program works under the supervision of a Registered Nurse (RN) and provides developmentally appropriate and age-specific direct and indirect patient care for participants in the adult care program which serves adults 55 years of age or older with one or more chronic or post-acute medical, cognitive, or mental health conditions.

Key Responsibilities:
  • Provides care in accordance with the established individualized plan of care for each participant based on diagnosis, orders, medical condition, psychosocial needs, physical condition, and other condition(s) pertinent to participant's specific situation.
  • Evaluates home care needs, available support, and compliance with the developed Plan of Care.
  • Seeks to improve or stabilize deterioration of the medical, cognitive and mental health condition.
  • Focuses on participants' lives and monitoring healthcare conditions while providing needed healthcare.
  • Utilizes medical knowledge and training to identify and determine observations and changes in conditions that are essential to be reported promptly to the RN and/or primary care physician.
Requirements:
  • Active California Licensed Vocational Nurse (LVN) license required.
  • Basic Life Support (BLS) certification and First Aid certification issued by the American Heart Association required.
  • Minimum of six months of experience as an LVN required.
  • Minimum one year of clinical experience in area of specialty preferred.
