Case Management
Schedule:
Monday - Friday, 8:00am - 5:00pm. No weekends, holidays, or on-call duties.
Responsibilities:
1. Develop and implement a holistic care plan that aligns with patient and family preferences.
2. Conduct regular assessments and evaluations to monitor patient progress.
3. Advocate for necessary resources and facilitate the removal of obstacles to care.
4. Maintain open communication with supervisors and healthcare team members to ensure effective execution of the care plan.
5. Serve as a resource for accurate medical record documentation and the appropriateness of services related to diagnoses and post-discharge treatment options.
Qualifications:
Licensure / Certification:
Must possess a Licensed Associate Professional Registered Nurse credential from the relevant state board prior to employment. BLS certification is preferred.
Education:
Must have a diploma from an accredited nursing program or equivalent professional licensure at the time of hire.
