Job Summary

Sun River Health Valentine Lane is seeking a highly motivated and detail-oriented Wellness Liaison to join our team. As a key member of our staff, you will play a vital role in promoting health and wellness services to our clients and community.

Key Responsibilities
  • Participate in outreach and educational events to promote health and wellness services
  • Serve as a liaison between clients, providers, and account management staff to ensure seamless communication and service delivery
  • Provide outbound reminder calls to clients prior to their scheduled appointments to ensure timely attendance
  • Assist clients in securing appointments for SRH Health Center services and conduct Health Home pre-eligibility screenings and intakes for eligible clients
  • Achieve intake monthly targets and provide necessary documentation and information to Departmental Health Home Manager
  • Participate in community events, such as health fairs, and conduct on-street marketing of flyers and service information pamphlets
  • Support the Health Center and Health Home by participating in Kiosk/Mobile events and engaging with the community to promote health and wellness services
  • Act as an initial service ambassador to promote and educate referral sources, clients, and community members about SRH services while providing exceptional customer service
