Job Title: Wellness Liaison

Sun River Health is seeking a Wellness Liaison to join our team at our Yonkers Park Care location. The Wellness Liaison will provide outreach and education services to coordinate intake and enrollment into Health Home and all Sun River Health service lines and programmatic services.

Key Responsibilities:
  • Participate in outreach and educational events
  • Serve as liaison between clients, providers, and account management staff
  • Provide outbound reminder calls to clients prior to their scheduled appointments
  • Assist clients in securing appointments for SRH Health Center services
  • Conduct Health Home pre-eligibility screenings and intakes for eligible clients
  • Achieve intake monthly targets
  • Assist with client outreach, education of SRH services to community, and linkage to Health Center and Health Home
  • Assist with health insurance application and transportation when needed
  • Provide necessary documentation and information to Departmental Health Home Manager
  • Participate in community events, such as health fairs, and conduct on-street marketing of flyers and service information pamphlets
Requirements:
  • Bachelor's degree preferred, High School Diploma/GED required with additional four years of relevant experience in customer service
  • Minimum 2-4 years of work-related experience
Hourly Rate:

$18.96/hour
