The Behavioral Health Home (BHH) Coordinator is a vital member of the Woodfords Family Services team, responsible for providing support, advocacy, and connection to services for children with disabilities and their families. This role involves assessing consumer functioning, developing and maintaining plans of care, and facilitating family/team meetings.
Key Responsibilities- Conduct regular assessments and evaluations to identify consumer needs and develop plans of care.
- Provide support and advocacy for children and families, connecting them to relevant services and resources.
- Facilitate and attend family/team meetings to ensure effective communication and collaboration.
- Develop and maintain relationships with community resources and agencies to ensure seamless service delivery.
- Bachelor's degree in Social Work or related field (LSW preferred).
- At least three years of experience in the field.
- Knowledge of regulatory guidelines and community resources.
- Valid driver's license and reliable vehicle.
- Competitive salary ($44,000-$46,000 annual)
- $1,500 sign-on bonus (paid at 6 months)
