Job Summary:

The Behavioral Health Coordinator is responsible for providing coordination of behavioral health services for enrollees in the Child Welfare Specialty Health Plan for Sunshine Healthcare.

This includes but is not limited to coordination of Multi-Disciplinary Staffings, ensuring children who require Suitability Assessments receive them in accordance with Florida Statute, provide clinical consultation and support and ensure funding authorization for children in STFC, STGC & SIPP levels of care are secured.

Key Responsibilities:

  • Assess and assist children in out of home care and provide care coordination of needed behavioral health services.
  • Provide identification of needed or necessary services and compilation of documentation necessary to ensure provision, including service authorization.
  • Ensure consistent and ongoing collaboration with Sunshine Health with contacting the applicable caregiver and/or enrollee.
  • Provide assessment of behavioral health needs, and identification of enrollees, who may benefit from Sunshine Health behavioral health care management services.
  • Provide coordination and facilitation of Multi-Disciplinary Team (MDT) meetings which are held in order to review behavioral health needs for children who are referred for behavioral health services, as well as for those who may require access to therapeutic placements and/or higher levels of care.
  • Participation in discharge planning activities, following children's admission into an inpatient hospital or crisis stabilization unit, to coordinate services and to ensure that the enrollee attends a seven (7) day follow up appointment.
  • Review of health and wellness reports (i.e., HEDIS Care Gap Reports), provided by Sunshine Health, which indicate services and performance measures that are due, or past due.
  • Provide follow up and outreach as needed.
  • Review case management reports/data, available within the Integrate system and/or the Sunshine Health Secure Provider Portal (SPP) and will communicate with the CBCIH Regional Coordinator and CBCIH Behavioral Health Care Consultant as needed, or upon request.
  • Ensure all care coordination activities are documented in compliance with CBCIH standards in Integrate application.
  • Act as the Point of Contact for receiving, coordinating and reviewing CBHAs from the Department of Children and Families Child Protective Investigators.
  • Participate in on call rotation to provide timely service.

Requirements:

  • Masters in social work or MA/MS in Human Services field required.
  • 1 to 3 years of experience in child welfare preferred.
  • Valid Driver's License - Essential.
