About the RoleAs a Social Services Assistant, you will play a vital role in promoting the well-being and quality of life for our patients/residents. Your primary responsibilities will include:
- Collaborating with our interdisciplinary team to identify and address the social, emotional, and mental health needs of our patients/residents.
- Developing and implementing individualized plans of care to meet the unique needs of each patient/resident.
