About the Role

As a Social Services Assistant, you will play a vital role in promoting the well-being and quality of life for our patients/residents. Your primary responsibilities will include:

  • Collaborating with our interdisciplinary team to identify and address the social, emotional, and mental health needs of our patients/residents.
  • Developing and implementing individualized plans of care to meet the unique needs of each patient/resident.
  • Providing education and support to patients/residents, families, and significant others to improve their quality of life.
  • Coordinating discharge planning activities, including referrals to community-based agencies and services.

Qualifications
