About the Role: The Geriatric Nurse Practitioner at PACE Southeast Michigan is responsible for providing high-quality medical care to our participants, focusing on the unique needs of older adults.

Key Responsibilities:

  • Conduct thorough assessments to identify potential health issues and develop tailored treatment plans.
  • Collaborate with our interdisciplinary team to coordinate care and ensure seamless transitions between providers.
  • Develop and implement individualized education plans to promote participants' understanding of their health and wellness.

Requirements:

  • A Master's degree in Nursing from an accredited institution.
  • At least three years of experience in geriatric nursing, preferably with a focus on frail or elderly populations.
  • Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions.

Estimated Salary Range: $140,000 - $170,000 per year

Working Conditions:

  • Variable working conditions with exposure to communicable diseases.
  • Daily driving within the PACE SEMI catchment area.
  • Frequent walking, bending, and lifting up to 40 pounds.
