Geriatric Care Specialist

PACE Southeast Michigan is seeking a highly skilled Geriatric Care Specialist to join our team. This is a challenging and rewarding role that requires strong clinical expertise, excellent communication skills, and a passion for delivering high-quality patient care.
Key Responsibilities- The successful candidate will be responsible for providing medical care to PACE Southeast Michigan participants, working collaboratively with physicians and other healthcare professionals to develop and implement comprehensive care plans.
- Assessing participants' health needs and developing personalized care plans to address their physical, emotional, and social well-being.
- Providing education and support to participants, families, and caregivers on various health-related topics, including disease management, health promotion, and wellness strategies.
- Maintaining accurate and detailed records of participant interactions, treatment plans, and outcomes.
- RN license in Michigan and NP licensure are required; Master's degree in Nursing with a focus on gerontology is preferred.
- At least 3-5 years of experience as an RN in a clinical setting, preferably in geriatrics.
- Ability to establish and maintain effective relationships with participants, families, and healthcare teams.
- Strong communication and interpersonal skills, with the ability to communicate complex information in a clear and concise manner.
- Variable work environment with exposure to communicable diseases, due to patient environment.
- May require frequent driving within the PACE Southeast Michigan catchment area, with possible exposure to extreme temperatures.
