Job Title: Medical Care Specialist

About the Role: We are seeking a highly skilled Medical Care Specialist to join our team at PACE Southeast Michigan. As a Medical Care Specialist, you will collaborate with physicians to provide medical care for participants, ensuring their health and well-being.

Key Responsibilities:

  • Conduct periodic assessments and physical examinations for acute visits.
  • Evaluate episodic illnesses and make referrals to other physicians as needed.
  • Provide primary care services, including health promotion and health screening.
  • Offer education to participants, families, and team members on various healthcare topics.
  • Communicate participant changes to the interdisciplinary team to ensure comprehensive care.

Requirements:

  • A Registered Professional Nurse with a current Michigan license and a Master's degree in Nursing.
  • Three to five years of clinical experience in geriatrics, preferably working with frail or elderly populations.
  • Ability to establish strong interpersonal relationships and apply teaching/learning theories.

Estimated Salary Range: $120,000 - $150,000 per year

Working Conditions:

  • Variable working conditions with exposure to communicable diseases.
  • Daily driving within the PACE SEMI catchment area.
  • Frequent walking, bending, and lifting up to 40 pounds.
