Job Summary:

As a Licensed Practical Nurse (LPN) with Affirmed Home Care, you will work closely with supervising physicians to assess and monitor patients' needs. Your primary responsibility will be to ensure that treatment and care meet the clients' needs, administer medications, and perform other related tasks. This is a Per Diem position that requires flexibility and a commitment to delivering high-quality patient care.

Responsibilities:
  • Assess clients' condition during every visit and track any observations
  • Take and record measurements of vital signs
  • Administer medication as prescribed by the client's physician
  • Dress or redress open wounds and assess progress of healing
  • Continuously educate clients, their families, and other caretakers on proper home health care strategies and procedures
  • Provide care as needed to keep the client comfortable
  • Listen to clients and be prepared to respond to concerns or requests
Requirements:
  • Current nursing license in the state
  • Physical & TB Testing within 1 year
  • Access to reliable transportation
  • Ability to work independently and with directions
  • Compassionate and friendly demeanor
