Key Responsibilities
Patient Care Delivery
As a Registered Nurse, you will be responsible for coordinating and delivering comprehensive care to patients and their caregivers. This includes:
  • Conducting thorough nursing assessments to manage patient and caregiver needs.
  • Developing, implementing, and updating care plans to ensure continuity and achievement of care goals.
  • Facilitating independence for patients and caregivers whenever possible.
  • Utilizing established protocols and guidelines for hospice care.
  • Providing effective education to patients and families, encouraging open communication.

Interdisciplinary Collaboration
Engagement in interdisciplinary group conferences and special care meetings is essential. Responsibilities include:
  • Building and maintaining effective relationships with team members.
  • Identifying patient and caregiver needs and referring them to appropriate team members.

Documentation and Accountability
Ensuring accurate and timely clinical documentation is crucial. You will:
  • Maintain professional competency and uphold the organization’s reputation.
  • Stay informed about current healthcare trends and incorporate new practices into your work.
  • Maintain all necessary licensure and certification requirements.

Minimum Qualifications
To be considered for this role, candidates must possess:
  • A Diploma or Associate Degree in Nursing (Bachelor's Degree preferred).
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN).
  • Current CPR certification; CHPN certification preferred.
  • A minimum of 1 year of relevant experience in inpatient units and 2 years in other nursing roles, with direct care experience for patients with life-limiting illnesses.
