Job Description:
We are seeking an experienced Intake Operations Coordinator to join our team. As an essential member of our intake team, you will be responsible for supporting community members by completing intake forms, scheduling meetings, and coordinating enrollment and retention activities.
Key Responsibilities:
- Administrative Duties: Completes intake assessments, verifies Medi-Cal and Medicare coverage, and updates participant information in PrimeSuite and PaceLogic.
- Communication: Answers phone calls, schedules meetings, and provides information regarding eligibility requirements and services.
- Coordination: Schedules Level of Care assessments, coordinates medical records, and assists with participant recruitment efforts.
- Documentation: Maintains intake files and reports, and updates New Enrollee Logs.
Requirements:
- High School Diploma or GED: Required
- 1 Year of Experience Working with Frail or Elderly Population: Required
- Proficiency in MS Office: Required
- Excellent Communication Skills: Essential
