  • We are seeking a skilled Pre-Enrollment RN to join our team at Center for Elders' Independence. As a key member of our interdisciplinary team, you will play a vital role in assessing the eligibility of individuals for enrollment into our PACE program.

Responsibilities:

  1. ">
  2. Conduct comprehensive nursing assessments and examinations to determine eligibility for our PACE program">
  3. Evaluate patient data, medical records, and input from other team members to complete Initial Level of Care (ILOC) documentation">
  4. Determine whether patients meet minimum requirements for skilled and intermediate nursing needs">
  5. Collaborate with our Intake Specialist to request denial of enrollment when necessary">
  6. Present assessment and ILOC data to our Interdisciplinary Team (IDT)">
  7. Complete assigned training and competency validation requirements">
  8. Perform other duties as assigned">

Requirements:

  • ">
  • BSN degree in Nursing, preferred">
  • Minimum 2-5 years of experience in community-based or hospital settings">
  • 1 year of experience working with the elderly population">
  • Bilingual: Spanish/English or English/Cantonese/Mandarin, preferred">
  • Clinical competency and technical skills for caring for seniors">
  • Knowledge of regulatory requirements for delegation of tasks">
  • Strong organizational, analytical, and problem-solving skills">
  • Able to function independently and guide other team members">
  • Familiarity with CMS PACE Regulations and Nurse Practice Act">
