Job Summary

The Intake Specialist I is a key member of our interdisciplinary team responsible for ensuring a seamless enrollment experience for our PACE participants. This role requires excellent communication and interpersonal skills, as well as the ability to work in a fast-paced environment.

Key Responsibilities

  • Coordinate and facilitate the intake process, ensuring timely and accurate completion of all necessary paperwork and documentation.
  • Work closely with the Enrollment Development Representative to identify and engage potential participants, and to develop effective outreach and marketing strategies.
  • Collaborate with the Interdisciplinary Team to assess participant needs and develop individualized care plans.
  • Provide education and support to participants and their families, ensuring they understand their rights and responsibilities in the PACE program.
  • Utilize the Client Relationship Management (CRM) system to track and manage participant data, and to ensure compliance with all relevant regulations and standards.

Requirements

  • Bachelor's degree in healthcare or a related field; or equivalent experience.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong organizational and time management skills, with the ability to prioritize multiple tasks and meet deadlines.
  • Ability to work in a fast-paced environment, with a high level of adaptability and flexibility.
  • Proficiency in computer applications, including spreadsheets, word processing, and CRM software.
