About the Role

In this critical position, the Home Health OT Assistant will deliver high-quality care to residents/patients, working closely with the occupational therapy team to develop and implement personalized plans of care.

Key Responsibilities:

  • Conduct comprehensive assessments to identify resident needs and develop targeted interventions.
  • Deliver evidence-based treatments that promote independence, safety, and overall well-being.
  • Collaborate with interdisciplinary team members to ensure seamless care transitions and optimal resident outcomes.
  • Communicate resident progress and concerns to the supervising therapist and other stakeholders.
  • Develop and implement educational programs to empower residents, families, and caregivers.

What We Offer:

  • $25-$32 hourly dependent on experience.
