Occupational Therapy Assistant

The Occupational Therapy Assistant in Home Health provides high-quality patient care, collaborating with the occupational therapist to develop and implement personalized treatment plans. • Perform thorough assessments and report findings to the occupational therapist, ensuring accurate data-driven decision making.
• Develop and implement individualized treatment plans, focusing on physical, cultural, and psychosocial needs.
• Communicate effectively with patients, families, and healthcare teams, promoting collaboration and continuity of care.
• Provide education and support to patients, family members, and caregivers, empowering them to take an active role in their recovery.
• Document and complete billing according to current standards and policies, ensuring seamless financial processes.
• Promote resident discharge planning, anticipating short-term and long-term needs, and collaborating with interdisciplinary teams to ensure successful transitions.

To be successful in this role, you will need:

• Current state licensure as an Occupational Therapy Assistant, in good standing.
• One or more years of experience in long-term care, preferred.
