HIV Navigator Community Outreach Specialist

About the Role:
The HIV Navigator Community Outreach Specialist will play a vital role in ensuring patient and community engagement through outreach and care coordination of patients living with or at risk for HIV. This position will improve community awareness, patient care outcomes, and quality of life for this focused population.
Responsibilities:
* Facilitate referrals for clinically indicated services outside the primary care office.
* Coordinate a collaborative and well-shaped intake service resulting in effective utilization and quality medical health services.
* Track patient follow-up and health outcomes.
* Document all in-person and telephone encounters for Adelante patients.
* Maintain regular contact with enrolled patients to monitor progress towards treatment goals and modify treatment plan as necessary.
* Connect patient with care team if concerns are noted due to changes clinical symptoms and treatment side effects or complications.
Requirements:
* High School Diploma or GED from an accredited institution
* Community Health Worker Certification preferred (or within 18 months of hire)
* Two (2) years of working experience in community outreach setting or clinical setting.
* Demonstrates cultural competency and ability to relate to people living with HIV/AIDS, gay/bisexual men, trans individuals, as well as individuals from diverse socio-cultural and socioeconomic backgrounds.
* Bilingual (English/Spanish) required
