Licensure Requirements
Multi-state licensure is highly desirable. An Immunization Certification from an accredited organization (e.g., APhA) is mandatory.
Key Responsibilities
- Assess physician medication orders for suitability regarding drug choice, dosage, potential interactions, administration routes, and stability. Provide recommendations for therapy modifications to physicians and facility personnel as necessary.
- Conduct monthly reviews of the medication regimens and clinical indicators for assigned residents, reporting any discrepancies to the Director of Nursing, Administrator, and attending physicians, accompanied by a written summary of findings and suggestions.
- Document all interactions with physicians, nursing staff, other healthcare team members, and patients/residents along with their families.
- Stay informed about pharmacy laws and federal and state regulations related to long-term care.
- Collaborate effectively as part of a healthcare team.
- Maintain expertise in pharmaceutical care practices tailored for long-term care residents.
Qualifications
- Bachelor's degree from an accredited School of Pharmacy or a Doctor of Pharmacy degree.
- Current pharmacy license is required; dual licensure is preferred.
- Preferred experience includes at least one year in acute care, long-term care, or home health settings.
- Consultant or Clinical Pharmacist experience is advantageous.
- IV clinical experience is preferred.
- Availability to work weekends or outside standard business hours when necessary.
- Physical ability to lift patient charts repeatedly and stand for extended periods.
- Strong customer service skills.
- Capacity to learn and comprehend state and federal regulations governing long-term care.
- Understanding of formulary management.

USD $57.00 - $59.00 per hour
